The COACH Program®
Being hospitalised for a heart attack, stroke or for other cardiovascular event can be a worrying time. The telephone-based COACH Program provides members with a personal coach for first six months following your hospital stay to help prevent or reduce your risk of further complications and take steps to improve your cardiovascular health.
What is the COACH* program?
Based on over 16 years of research, The COACH Program has been proven to help people with cardiovascular disease meet the recommended National Heart Foundation and National Stroke Foundation targets for specific risk factors, reducing their chances of future complications.
After leaving hospital, you’ll be partnered with a qualified dietitian trained in cardiovascular disease management. Your coach will work closely with you to help achieve your relevant risk factor targets including:
- smoking, nutrition, alcohol, physical activity and weight (known as lifestyle or behavioural risk factors)
- lipids, blood pressure and diabetes (known as biomedical risk factors).
Am I eligible for the COACH Program?
Bupa members with hospital cover are fully covered for the COACH Program. If we think you’ll benefit from the COACH program, we’ll speak to you shortly after you’re discharged from hospital to see if you’d like to take up the service. If you have recently been admitted to hospital for a heart attack, stroke or for peripheral vascular disease and would like to be contacted about whether you are eligible for this program, please complete the Member Health Support form.Download the COACH Program brochure
