Delivered by Ramsay Home Health, the Virtual Heart Failure Service offers:

The Ramsay Home Health Virtual Heart Failure Service is available to eligible Bupa Health Insurance customers* at no additional cost.# Talk to your GP or treating heart specialist to determine if this program is right for you.
What does the program provide?
For many people with heart failure, regular hospitalisation is a fact of life. However, it doesn’t always need to be this way. While people with severe CHF will usually require regular hospital care, many other hospitalisations may be avoidable.
The Virtual Heart Failure Service aims to reduce the amount of time you spend in hospital as a heart failure patient. This means fewer visits and shorter stays when you are admitted. The service strives to achieve this goal by providing regular monitoring of your vital signs and well-coordinated allied health care, as well as helping you better understand your symptoms. This can help you and your healthcare team notice any changes in your condition and manage them early, which may prevent an unnecessary hospital visit.
Every participant in the Virtual Heart Failure Service will receive the following:
1. Technology and wearables to monitor your heart failure
You receive weight scales and a Bluetooth-enabled blood pressure monitor and pulse oximeter. All this technology is yours to keep.+
2. Remote monitoring and data analysis
Your blood pressure, weight, pulse, and oxygen saturation are monitored and analysed remotely by a team of cardiovascular nurses. You also fill in occasional health surveys from Ramsay Home Health to provide further insights into your condition and overall health.
3. Virtual scheduled consultations with specialist nurses and allied health professionals
You can schedule chats with nurses and allied health professionals about your condition from the comfort of your home, using video consultations and text chats.

Program eligibility
If you meet the following criteria, you may be eligible to take part in the Virtual Heart Failure Service:
a) You are a Bupa Health Insurance customer with a hospital policy that includes unrestricted cover for heart and vascular system – check what you’re covered for or contact us
b) You can access the Virtual Heart Failure Service in available areas (NSW, Vic, Qld, SA, and WA).
c) You have chronic heart failure and meet all other clinical requirements outlined during the screening process.

#Depending on your level of cover some out-of-pocket costs may apply, like hospital excess or co-payments, fees for medical tests or specialist consultations not covered by bulk billing, and medication. Additional expenses such as having a compatible smart device and reliable phone and internet connection also apply for virtually delivered care. Any technology you keep beyond the program duration will need to be maintained at your own cost. +You receive a Bluetooth-enabled blood pressure monitor and pulse oximeter and you will need a compatible smart device to use these. Instructions and training will be provided by the provider.
