To help manage your health, we'll provide a benefit towards some of the costs of eligible gym memberships, personal training and mole mapping^ services (including skin checks) at Bupa recognised providers*.

Eligible Gym memberships & personal training fees
Gym membership and personal training fees are only claimable when the exercise program is recommended by your health professional to address or improve a specific diagnosed health or medical condition – for example, diabetes. A health professional (such as your GP, medical specialist or exercise physiologist) must complete the Health Management Gym and PT Medical Form on your behalf. We will only pay benefits for Bupa recognised* Gyms and Personal Training providers. On selected covers only, waiting periods, yearly limits, benefit claiming restrictions, fund and policy rules apply.
Follow the below steps:
Step 1: Ensure your gym or personal trainer is a Bupa recognised provider*. Please call us on 134 135 to check if your provider is recognised with Bupa.
Step 2: Download the Health Management Gym and PT Medical form here.
Step 3: Provide the Health Management Gym and PT Medical form to your health professional (e.g. GP, medical specialist or exercise physiologist). Both you and your health professional will need to complete and sign the form.
Step 4: Make payment for your gym or personal trainer and obtain a receipt. Please note that in order for your claim to be accepted, the date on the receipt must be after the date on the signed form by your health professional. Note that Bupa does not pay benefits for trial periods, joining fees, casual attendances or bundled passes at a Bupa recognised provider*.
Step 5: Log into myBupa to submit your claim. You will need to attach your completed form and receipt.
Please note: The Health Management Gym and PT Medical Form is valid for 12 months from the date signed by your health professional. You will need to provide a new signed form every year to continue to claim. Please refer to FAQs for further information.

We will pay benefits on mole mapping^ services (including skin checks) completed by a Bupa recognised provider*. The process is easy just follow the below steps:
Step 1: Ensure your local mole mapping clinic (or skin checks clinic) is a Bupa recognised provider*. Find your closest recognised provider at Mole Map.
Step 2: Visit the clinic for your appointment.
Step 3: Pay for the service and obtain a receipt.
Step 4: Log into myBupa to submit your claim and attach your receipt.
