NZ Visa Medical Assessment checklist Important: Identity documents presented at the time of the health examination must be originals. You will not be able to proceed with your appointment if you present a digital copy or a photocopy of your passport/identity document.

Current valid original passport

If your passport is not available, the following original identity documents are accepted

  • New Zealand Certificate of Identity with photo
  • Refugee travel document with photo

Eyeglasses or contact lenses if you need them for the eye test.

Information about any long-term prescription medication that you take. Copies of medical reports about any current or previous medical conditions. If you're pregnant, you must bring a certificate or letter from your doctor to confirm this. If you have an Immigration Adviser, you must bring your completed 'Immigration Adviser Details' form.
