What to bring

NZ Visa Medical Assessment checklist

Important: Identity documents presented at the time of the health examination must be originals. You will not be able to proceed with your appointment if you present a digital copy or a photocopy of your passport/identity document.

Current valid original passport

If your passport is not available, the following original identity documents are accepted

New Zealand Certificate of Identity with photo
Refugee travel document with photo

Eyeglasses or contact lenses if you need them for the eye test.

Information about any long-term prescription medication that you take.

Copies of medical reports about any current or previous medical conditions.

If you're pregnant, you must bring a certificate or letter from your doctor to confirm this.

If you have an Immigration Adviser, you must bring your completed 'Immigration Adviser Details' form.
