Canadian Visa Medical Assessment checklist Important: Identity documents presented at the time of the health examination must be originals. You will not be able to proceed with your appointment if you present a digital copy or a photocopy of your passport/identity document.

Current valid original passport and a colour photocopy of your passport’s bio page, if available.

If your passport is not available, the following identity documents are accepted:

  • A driver's license issued from one of the following countries (if you're still in that country)
    • Australia
    • Canada
    • New Zealand
    • United Kingdom
    • United States
  • National Photo ID card
  • Seaman's book
  • Refugee travel document
  • Red Cross travel document
  • UN Laissez-passer
  • Organization of American States travel document (OAS)

For children and students who don't have one of the identity documents listed:

  • Please provide a birth certificate and/or School/College/University ID
  • The accompanying parent/guardian must present one of the ID documents listed above
IME/UCI/UMI number, if available

Eyeglasses or contact lenses if you need them for the eye test

Information about any prescription medication that you take Copies of medical reports about any current or previous medical conditions
