Australian Visa Medical Assessment checklist Important: Identity documents presented at the time of the health examination must be originals. You will not be able to proceed with your appointment if you present a digital copy or a photocopy of your passport/identity document.

Current valid original passport

Or an original passport which has expired within two (2) years of the appointment date (unless exempted by the Department of Home Affairs).

If your passport is not available, the following original identity documents are accepted:

  • Current Australian driver's licence
  • Australian State or Territory photo ID card
  • Immicard
  • Current valid emergency travel or temporary travel document

If you do not have an acceptable passport, please visit the Department of Home Affairs' website for more information.

HAP ID.

An eMedical referral letter, a Health Examinations list or other equivalent documentation provided to you by the Department of Home Affairs.

Eyeglasses or contact lenses if you need them for the eye test.

Copies of medical reports about any current or previous medical conditions.

If you have previously been investigated or treated for Tuberculosis (TB) please bring prior records relating to this treatment such as specialist reports of x-rays.

Please bring any other specialists reports you may have and,

details of your current medications
