Update your details
It is important that during your stay in Australia you keep us up-to-date by advising us of any changes to your details.
If you would like to change your details (address, phone number, email) please tick “Change My Details” on the front page of the form, and then complete Section B and C.
If you would like to add new members to your Family Membership, please tick “Add Someone to My Membership” and complete Section B, E, and F (if required).

Authority form
This form may be used by the policyholder to grant another person ('The authorised person') authority to access their policy. Authorised persons must be over 18 years of age.
