The COACH program®

Being admitted to hospital following a heart attack, stroke or for other cardiovascular event or procedure can be a worrying time. The telephone-based COACH Program® can help members through this challenging period.

The COACH Program® involves regular phone contact with a qualified dietitian trained in cardiovascular disease management - a personal 'coach'. Over a six to nine month period following hospitalisation for a cardiac or stroke related illness, coaches work closely with members to identify steps they can take to reduce the likelihood of future complications.

This includes achieving the recommended National Heart Foundation and National Stroke Foundation targets for risk factors such as cholesterol, blood pressure, smoking, nutrition, alcohol, physical activity and weight. When these targets are achieved, the chances of cardiovascular events recurring may be reduced by up to 80 percent.

The COACH Program® is fully covered for Bupa members with hospital or hospital and extras cover. Members who might benefit from the program may be contacted by phone shortly after they have been discharged from hospital to let them know that The COACH Program® is available if they wish to use it.
